Kidneys?

This thread should probably go in the Health/Nutrition section but I thought I'd get more replies here so here goes. I just completed my first half-iron distance race yesterday and had a problem 1/2 way through the bike. I started having a little pain in what I assume was my kidneys. It was that same feeling that you have when you wake up in the middle of the night because you have to pee, but just too tired to get up until it finally starts to hurt. But I never really felt like I had to pee. When I stopped, I did pee but I never felt like I had to. I feel like I hydrated and fueled well but just don't understand the pain. At first I thought it was just that my low back was sore since this was my first tri at this distance but when I got to the run I could really feel it each and every time my feet hit the ground. It was a really hilly couse so I had many moments where I had to walk. The pain continued for about three hours after the race on the drive home and I could feel it with each breath. Altogether I only peed three times during the race but the color was still pretty clear so I figured all was fine. Temps were in the low to mid 70s. My AVG HR for the race was 159. Had 1 gel prior, 3 gels on bike, 4 bananas, 1 orange, 1 apple, a cookie and 6 e-caps on the run along with 3-4 oz. of either Gartorade or water or both at every aid station. Any ideas?

I just wanted to say that if you are sure it was your kidneys you should go and see a doctor. (well I´m a bit paranoid when I suffer from anything but... I prefer to have everything under control)

Interesting question...!!!My good friend, first of all, big congrats on your race, great achievement.
Now, kidney pain, is something that might happen to an athlete, and particulary...and this is no joke, I suffered the same pain this very weekend.
But from a more medical point of view, kidneys only hurt, if they are swollen, because they can´t drain the fluid (pee) out, like with a congenital malformation, or a STONE!
But now...there are other semiological points that are important, and the whole clinical picture points towards something away from your renal system. Now, two questions to you my friend:
1. Do you have previous history of renal stones, or in your family?
2. Did the pain irradiate somewhere else?
When you pass a kidney stone..anecdotically, and correct me any trifueler who suffered this, but is the worst pain, particulary for men, that a patient ever experienced...terrible, that can actually lead you to shock due to intenese pain. But you mentioned that your urine was clear...mmmm, usually when we pass a stone, it lacerates the channel so we get bloody urine....Now young athletes in their 20´s or 30´s are said to be statistically more prone to form stones, due to electrolyte and liquid balance during endurance trainning and/or events. But as fas as your post was, seems to me you hydrate pretty well, proof of that was the color of your pee...
Other medical entities, that might cause pain in the kidney are way more rare to occur...and other symptoms might already occur, that is not your case.
I would say try to asses more your lower back...that can cause a similar pain, that might be incapacitating, and that can scare you to dead because confusion that you are passing a stone or something...
Now, the best aproach to this, is get an appointment with your family doc or primary care, tell him/her your symptom and that it actually happened during an endurance event. Get a urine analisis, and maybe some blood work just to be sure, would not hurt.

No family history of stones and no unusual pain anywhere else. Sorry for being too graphiic but the first time I went to the porta-john in T2, it did burn a little. The other times it was OK. I've only had 2-3 other rides at or above this distance so maybe that had something to do with it. From the descriptions I've heard from others who have had Kidney stones, this was definitely not that kind of pain. Just something that wasn't right.

Well, the clinical picture sounds really interesting...there is nothing more appealing for a clinitian than a tricky case...
Now, my best advice for you...since you had some burning and you describe it as a different pain...get to your primary care, he/she might run some blood work and urine sample....
Let me know how it goes....now as a precaution measure, drink lots lots of water...more than anything else....to keep those kidneys flushed with fluid and taking you to the bathroom frecuently till' you get check...
