You mean relating to tri's or life in general?:)
We all make sstupid mistakes in Training and racing....
"Oops forgot to take off my helmet"
"Where is that front wheel?"
Stuff like that...
I't rule # 6 of life...."Everyone F@#*%s up...alot. Although we don't like to admit it.
Last weekend I finished a long ride, got back to the parking lot,put the bike on the roof rack and drove off..thankfully it was only two minutes before I realized I left the front wheel there and had to go back for it...! (Doh!)

My husband, a hardcore triathlete for many years, has the nick name of The Space Cowboy. I have to watch that man's every move or he would be dead in a week. He does not have a worry bone in his body and for hours after a work- out he really is not all that alert.
The man Anton says we all do it, I do not admit to it, but it is how much importance we put on these regressions that counts. S.Cowboy is always whistling that tune "don't worry be happy." What a jerk!
Oh yes, I have that problem as well. I'll be out on a ride and all I can think about his house work and what I'm making for dinner, etc. Sad thing is that I'm only 25. :(

Judging from the riding pictures you recently posted it probably has to do with that big smile you have while you are riding and all the air flowing into your mouth. Too much oxygen flowing to your brain............not to mentioned bugs in the teeth.

are you calling me an airhead?

I totally have that problem, often more so in long training weeks, so I'd say yes, training does contribute.
For example, after bike commuting more than usual, and getting in runs and swims last week: I got of the train, biked over to my car, and put the bike in the back. Feeling like I was forgetting something, I looked around. Nope, everything was in the car. So I climbed in and turned it on. Huh... it feels odd.... Ooooooooooooh I still have my bike shoes on... and my gloves... and my helmet! I returned to the back of the car and removed helmet and gloves and put on my normal shoes.

LOL!!! I keep going into rooms and forget why I entered. I also bought groceries, and left them at the store. shhhh don't tell my family. I am sure it has to do with the red blood cells. But seriously, there are blurry periods when I train harder.

I would never intentionally call you an airhead Blitz. I have the utmost respect for spacey, blonde, Swiss women who forget why they walk into rooms.

You are definitely not alone. I forget things ALL THE TIME! I have to make lists and I keep a day planner and write everything down. There's nothing worse then getting somewhere, grocery store, mall, etc., and then standing there in the doorway trying to remember why you're there.
Or even worse, driving for ten minutes before you realize you'r going in the wrong direction

I do that all the time any way....the big one is "where did I leave my coffee cup?"
There is also:
me: "damn it I can't find my phone!"
holly: "umm... you are talking to me on it"

I haven't noticed that my short term memory got any worse since I started training more. It was bad enough before. I am the guy that has left the remote control in the freezer. Given that, there is certainly some overlap of mind and body fatigue. The mind is a complicated thing however, and mid-distance workouts certainly help relieve some types of mental fatigue. We all know that feeling of being recharged after a workout. This only works to a point, and I think most people on this board are past that point. Even so, I think it really just comes to balancing all the different stressors, getting proper rest, and periodically shifting mental focus throughout the day.
Remote control in the freezer. I've not even managed to do that yet. Today I Locked the front door and looked down and saw that I had my slippers on. Think I should not train for a while or get a cat scan.

Does anyone else have problems concentrating, make stupid mistakes, forget and so-forth and is under the same suspicion that it has to do with training? Or am I just getting old?
