dehydration, heat stroke? WTH happened?

Just typing this out to try to figure out what happened this morning. I went for a 6 mile run, follwed by a 40 mi bike ride. My run partner joined me at mi 20 of the ride and we decided to run 2 mi after as a quick brick. (I normally do long run Sunday, followed by an hour or so ride, but I decided to do my long ride monday instead.) I had my regular sips of water at my regular intervals on both the run and bike. I finished 2 bottles of g2 and a bottle of gatorage mixed with water (along with some clif shot gels and a fig newman or 2). This is always what I do. I have never had any issues with hydration or fueling. I felt somewhat fatigued, but when I got in the calories, I felt better.

Once we got off the bike, and started running, it went downhill fast. I had a side stitch, I figured from drinking too much, I was going SLOW! and getting slower. We turned around at about a mile and I told my running partner to go ahead, I would walk. I started walking, and I became short of breath, my ears started ringing and my vision went to black and gold dots. I sat on a bench and tried walking again after a bit, but it was worse. Eventually, I saw a woman parking by the street and asked if she could drive me to my car.

It is now in the 80s and 100% humidity at 5am (when I started), but I am used to that. One friend said that Gatorade did not sufficiently replace electrolytes, one friend said it was the extreme heat, one friend said I didn't hydrate enough the day before...but NOTHING was different than what I normally do except the workout. (and I wouldn't say it was "harder")

Sorry for the rambling, but I would hate for this to become a trend. It scared the daylights out of me. Any thoughts from the experts here?

Salt caps? Gatorade and water alone wont cut it on hot humid days with extended workouts. You'll need to maintain electrolyte balance with caps to effectively allow your system to absorb liquids. Enduralytes, SaltStick and S-caps are some of the familiars. S-caps have the highest amount of sodium and are buffered to help out with stomach issues Some use Nuun tabs and water.

Experiment around and see what works for you on long training days. A cap every hour or so after 2.5-3hrs of extended workouts seems to work for me.

+1 to the S-caps (Succeed! caps) Every hour on the hour. I'm going on 15 years as a Succeed! Caps user. Never had a problem. As endurance athletes I think many of us walk around in a state of chronic dehydration. It bears paying close attention to everyday when you're training in hot humid conditions. It WILL come around to bit you in the ass if you're not careful. I went out for a long run in nasty conditions two weeks ago and didn't supplement. Bam! Felt awful for most of it

How long has it been hot out by you? Your body takes time to adjust to heat (increased water retention among other things). Do you work in an air conditioned office all day? That could effect how long it takes your body to adapt to heat/humidity. It could also have been from low electrolytes like what other people have said. OR it could of just been a bad day brought on by a combination of factors.

+ 1 to the salt caps. Check also lava salts. I like it.

+1 to the above. I use E-caps from Hammer. Even with those, bonking still happens, especially on humid summer days. Did you look at what you ate and drank the day before?

crazyquick, it has been hot and humid like this for a good 2 months.
Garen, the day before, I spent some time at the pool with my kids (hot) and walked the dog for an hour (hot), but I do that every Saturday.
I have never taken any sodium tablets, and yet, have never had a problem. Maybe I'll try that NUUN stuff in the water. I just assumed G2 was the same electrolyte stuff. I'll have to do better label reading.
I did a 13mi long run this morning, in the same conditions, and I was drenched, but feeling okay at the end of it.

I'll just say BE CAREFUL!

I had a heat-stroke back in the Army and almost died. Recovery was about 6 months. It's nothing to fool around with.

Sharon, glad you bounced back today. You did the right thing, the other day, by stopping. It is one thing to keep pushing when your legs are shot, but when your head starts to get funny it is time to stop or take a break.

You're training in humid sub-tropical Houston. That's some nasty heat exhaustion every long day. Take all the salt you want. It won't change your Houston weather.

Hello trimon:
From what I read, I would say from a clinical point of view, that your problem was dehydration and low glucose levels, that with the heat of the summer made you feel the symptoms that you had. I have two questions:
1.Did you have chills and stop sweating; did you feel nausea?
2.Why you drink G2, are you trying to control your weight, even while you are training?

The first question is to really try to rule out overheating, moving towards a heat stroke.
The second one, is to maybe point out that your caloric intake does not neet to be reduced to those light sports drinks.

For the symptoms you describe, more than likely your blood preasure dropped, and also was acompained with a low blood glucose level.

Try to keep a sort of diary to recall what things you did, and did not, and correct them.

Hope that you are doing much better.

thanks, Dr. Rios.
1) No, I didn't have chills or stop sweating, in fact, when I sat on the bench to rest, I took notice that I was pouring sweat, literally dripping.
2) I drink G2 because I thought it was more an electrolyte replacement drink (I may be mistaken), I use that in combination with regular Gatorade among other caloric replacement foods, gels, chews (as I started in the original post).

I am doing much better, thanks. I imagine it was a combination of several things, but stressful regardless. I hate that feeling of losing control of your body...can't imagine many triathletes enjoy that. I will try a shorter brick wed or thurs just to boost my confidence, as I have a sprint this Sunday, and now I am all freaked out about it. :(

jtrimom:
I am so glad you are doing better. By the way...I do train too in Houston ;-)

Now, about your symptoms lets talk a bit more:

If you did not have chills, or abscence of sweat, nausea, then I was not a problem of over heating, those are the cardinal symptoms of over heating and heat stroke. In fact if you were pouring sweat sounds more like a case of "bonking"(low glucose) and probably dehydration (low blood pressure). One thing that really makes a world of difference is prehydrating a bit agresively during these hot humid days here in the Gulf of Mexico.
With all due respect to the sport drinks manufacturer, G2 is a light option that goes against the concept of replenishing calories and lytes for athletes, more useful for regular people than for triathletes and serious runners. With all the training you do, I think you might not need to watch your caloric intake cutting down what comes in your beverage.

Healthwise, my recomendation is to try to hydrate better before your training during this hot months of summer here in the Gulf. Increasing sensively your salt intake will help too.
Usually what I do, is mix one tiny measure of endurolyte powder with every liter of gatorade I bring for my runs.

I am glad to read you are doing much better and best of luck on sunday.
