Okay, so I did my longest run so far last weekend (20 mi) and had a some pain in my joints during the last 4-5 miles and afterwards. Not unexpected since it is a looooong one for me! I took 3 days off, until the pain subsided, then yesterday went out for an easy 6-7 mi jog and at 4.5, my right ankle started protesting pretty bad. Ran/walked back...

The pain is centered around my ankle and primarily resides in the achilles tendon to top of foot area and around the ankle bone. Any thoughts on what, if anything, it may be? I'm thinking it's probably an over-training injury and plan to bike/swim the rest of the week and not run at all (biking does not hurt it as I commute to work everyday.) Thoughts/suggestions???

Thanks!!

Doctor...now. If the pain was bad enough that you had to cut the run short, don't take any chances.
Better to have a good handle on it than stumble around guessing what it might be while icing and sucking down NASIDS.Running is load bearing so sure that will hurt, but biking may be adding to the damage even though it doesn't hurt...get it checked.

+ 1 Go see a doc. A doctor's knowledge can't hurt (no pun intended).

Good call. Inability to move freely with pain is a sure sign of an issue, even if it isnt in the immediate area of pain. It could show an imbalance in some are of you body around the immediate connective tissue.

Good luck with the doc.

You can get it treated and take time off, but you should also look at possible causes. I'd look at what your mileage progression was leading up to the 20 miler. Did you get in several months (yes, months) of running with varying distances 10, 12, 14, 16, 18 mile runs? Next, I'd look at your running surface. The "around the ankle bone" suggests maybe an uneven running surface was a factor. Do you trail run? (I think trails present more threat to injury than injury prevention.) Do you run on a cambered-road? Run as much as you can on the crown (which around here means running in the middle of the road). Finally, do you have the right type of shoe. The location of the injury sounds like it could be attributed to lateral movement of the ankle-area. You may need stability and motion control shoes. After you re-hab, stop by a good shoe store that offers 'gait analysis' on a treadmill with video and double-check that you have the right type of shoe.

+1. Professional advice and experience have taught me that injuries to the knees and ankles are typically not problems within the knees and ankles, but rather the supporting structure. Even if you're running on a flat surface, weakness or tightness of the leg muscles and hips will cause lateral movement at the joints (running on a camber compounds it). If you heap the miles on too quickly, stress-induced injuries are inevitable - but misalignment can catch up with you even if your runs are consistent.

You can stave off a lot of overuse injuries simply by staying loose in between sessions, stretching thoroughly after your workout, and sticking with a recovery regimen (e.g. icing, compression, etc.). As always, if it hurts, stop. Seeing the doctor is the best move, and if you can get an independent second opinion (be it a PT or a dr.) you'll be doing yourself an even greater service. In my last bout with running issues, I went through 2 orthopedists, 2 PTs, and a chiro before I found a PT (60 miles away) that was able to diagnose the problem and get me back on the road. Coincidentally, he is a 3x Ironman. Good luck!

Thanks eveyone. I'm seeing the doc in an hour. I did a gait analysis at a place my sister uses in San Diego about 2 years ago and have been buying their shoe recommendations since then. Good point about the road surface. We ran on a variety of surfaces that day...the roads in Japan actually suck for running....lots of patches, generally the pavement follows the contour of the ground (ie they don't flatten it like we do), etc. Maybe the lateral issue is the problem. As far as the amount of running, I've been consistently running/increasing mileage since Sept and the longest regular weekly runs (based on Runners World 1/2 and Marathon tool) have been between 15-18 on Sundays (for the last month or so. So, the 20 was 3 miles farther......hmmmmm. Got my finegrs crossed for some "not so bad" news :)

Okay....nothing fractured or broken. PT thinks it an overstress issue which was aggravated by some of the uneven surfaces during the 20 miler. So, 20 min of HIVOLT and a cold boot for everyday over the next week. And.... no running :( Biking and swimming are fine she said but, the running has to stop for a week and then SLOWLY back into it......just need to be strong for the Tokyo Marathon on Mar 22......now for the hard part.....doing what she says!
